Transitional Liaison Licensed Practical Nurse - Hybrid

As a Transitional Liaison LPN/LVN, you will contribute to our success by identifying patients who may qualify for hospice care, following the hospice referral, eligibility, and documentation process, and assessing patients' physical, emotional, social, and spiritual needs. You will work with care teams to coordinate patient transitions and services, meet with patients and caregivers to discuss care goals and service options, and explain hospice services so patients and families can make informed decisions. Additionally, you will educate staff and referral partners on hospice eligibility and the transition process, and support the hospice team by coordinating and assisting with admission visits. Requirements
- Current state license as Licensed Practical Nurse/Licensed Vocational Nurse
- Graduate of an accredited LVN/LPN Program
- 3+ years’ experience in a clinical care setting
Nice To Haves
- Experience within Home Health or Hospice is preferred
