About The Position
Centurion is the provider of comprehensive healthcare services for the Indiana Department of Correction. This role is for a Transitional Healthcare Facilitator at New Castle Correctional Facility in New Castle, Indiana. The facilitator will be responsible for developing and implementing transitional healthcare plans for incarcerated individuals who are scheduled for release. The position involves coordinating with the Indiana Department of Correction and other state and local agencies to ensure pre-discharge planning and preparation, and to provide necessary services and resources to individuals re-entering society.
Requirements
- Bachelor’s degree in human services, social services, or criminal justice field
- Two years of experience working in a correctional setting
- Experience with/knowledge of community and state programs and services
- Must maintain current tuberculosis screening documentation
- Must maintain active CPR and Automated External Defibrillator (AED) use certification
- Must be able to pass background investigation
- Must be able to obtain agency security clearance where applicable
Nice To Haves
- Two years of experience in re-entry services or release planning preferred
