About The Position

Centurion is seeking a full-time Transitional Healthcare Facilitator Float to join their team, providing support at correctional facilities and parole district offices throughout Southern Indiana. This role will primarily support Wabash Valley Correctional Facility in Carlisle, Indiana, and float to other facilities as needed. The Regional Transitional Healthcare Facilitator is responsible for developing and implementing transitional healthcare plans for incarcerated individuals scheduled for release. This includes coordinating with the Indiana Department of Correction and other state and local agencies to ensure pre-discharge planning and preparation are accomplished. The facilitator will also engage with recent parolees post-release to support their continuum of care, provide education and support, and connect them to community resources for a successful transition. The role involves assigned projects and travel to correctional facilities and parole district offices as needed throughout the Central/Southern Indiana Region.

Requirements

  • Bachelor’s degree in human services, social services, or criminal justice field
  • Two years of experience working in a correctional setting
  • Experience with/knowledge of community and state programs and services
  • Must maintain current tuberculosis screening documentation
  • Must maintain active CPR and Automated External Defibrillator (AED) use certification
  • Must be able to pass background investigation and obtain agency security clearance where applicable

Nice To Haves

  • Two years of experience in re-entry services or release planning preferred

Responsibilities
