About The Position
Centurion Health is seeking a full-time Transitional Healthcare Facilitator Float to join their team. This role provides support at correctional facilities and parole district offices throughout Southern Indiana, primarily supporting Wabash Valley Correctional Facility in Carlisle, Indiana, and floating to other facilities as needed. The position involves developing and implementing transitional healthcare plans for incarcerated individuals scheduled for release, coordinating with the Indiana Department of Correction and other agencies for pre-discharge planning, and engaging with recent parolees post-release to support their continuum of care, provide education, and connect them to community resources for a successful transition. The role also involves travel to correctional facilities and parole district offices within the Central/Southern Indiana Region for assigned projects.
Requirements
- Bachelor’s degree in human services, social services, or criminal justice field
- Two years of experience working in a correctional setting
- Experience with/knowledge of community and state programs and services
- Must maintain current tuberculosis screening documentation
- Must maintain active CPR and Automated External Defibrillator (AED) use certification
- Must be able to pass background investigation and obtain agency security clearance where applicable
Nice To Haves
- Two years of experience in re-entry services or release planning preferred
Responsibilities
- Development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release.
- Coordination with the Indiana Department of Correction and other state and local agencies to ensure pre-discharge planning and preparation.
- Engagement with recent parolees post-release to assist with and support the continuum of care.
- Providing education and support to released offenders.
- Connecting released offenders to community resources to enable a successful transition into their community.
- Assigned projects at and travel to correctional facilities and parole district offices as needed throughout our Central/Southern Indiana Region.
