About The Position

BAYADA Home Health Care is hiring a Transitional Care LPN for its SEAL (Skilled Elite Adaptable Leaders) team in the Twin Cities. SEAL team members are experienced field employees who help bring medically complex referrals home, support training and up-coding of nurses, and fill shifts for clients in need. The role floats to areas of greatest need and is not a full-time travel position. Nurses receive a higher rate of pay and enhanced benefits in recognition of scheduling flexibility, travel, and the acute level of care provided. The team’s goal is to provide adequate staffing, potentially up to 24/7, support new referrals, fill staffing gaps for existing clients, and help grow the field team by training new nurses. Care may include infant, pediatric, and adult clients, as well as tracheostomy and/or ventilator care and IVs. Team members may also attend school with clients when applicable. BAYADA’s mission is to help people have a safe home life with comfort, independence, and dignity. Its work is guided by The BAYADA Way, the company’s philosophy, mission, vision, beliefs, and core values.
