About The Position

The Transition Specialist is responsible for providing comprehensive, culturally competent services to individuals detained in city jails and upon their reentry to IMPACT. This role involves case management, transition planning, and performance management to ensure participants receive the necessary support for a successful transition back into the community.

Requirements

  • Bachelor’s degree in human services, psychology or special education, or a related field from an accredited college or university.
  • Minimum of 4 years of working with the population, plus an HS diploma or equivalent instead of a bachelor’s degree.
  • Must be able to receive entrance clearance to Rikers Island.

Nice To Haves

  • Experience working with formerly incarcerated individuals preferred.
  • Case Management experience preferred.
  • Bilingual (English/Spanish) preferred.

Responsibilities

  • Conduct thorough assessments and develop individualized, person-centered Comprehensive Service plans for 100% of participants on caseload using a motivational interviewing process.
  • Provide crisis intervention and engage in safety planning.
  • Provide individualized services to 100% of participants on caseload from orientation to exit.
  • Offer evidence-based clinical support as needed.
  • Facilitate care from Rikers through successful reentry in the community, ensuring connections to appropriate internal and external services.
  • Work in conjunction with Reentry Mentors and Reentry Coordinator to ensure seamless transition and continuity of care for 100% of participants on caseload.
  • Conduct quarterly presentations of reentry services offered in the jail.
  • Develop individualized, person-centered transition plans for 90% of assigned houses through thorough assessments and motivational interviewing.
  • Expedite communication and/or case conferences with DOC, CHS, jail-based providers, defense teams, community partners, and positive supports.
  • Maintain regular contact with participants to meet program standards and achieve program retention for 100% of participants on caseload.
  • Sustain open communication with program participants through face-to-face contacts, phone conversations, emails, and text messages.
  • Provide outcomes reporting monthly.
  • Maintain 100% HIPAA and Informed Consent Compliant Documentation.
  • Manage participant files, including progress notes and reports on progress toward goals.
  • Provide weekly reports to the team and Program Director.
  • Participate in regularly scheduled supervision and ongoing training.
  • Attend meetings and participate in professional development training offered.
  • Enter case notes for every job seeker in FedcapCARES daily.
