About The Position

The Arc of Madison County, Inc. is seeking a Transition Specialist/Job Coach to instruct Pre-Employment Transition Services (Pre-ETS) classes, including Social Skills, Workplace Readiness, Self-Advocacy, and Job Readiness. This role provides crucial support to individuals with intellectual disabilities, enabling them to become active and valued members of their community. Responsibilities include conducting assessments, ensuring accurate service billing information, collaborating with program coordinators to meet funding requirements, assisting with facility and vehicle maintenance, and providing ongoing training and support for individuals in various employment settings, including small group and competitive employment. The position also involves providing extended services to individuals who have gained and retained community employment.

Requirements

  • High School diploma or GED is required.
  • At least one year experience in day habilitation or residential program required.
  • Possess basic computer skills.
  • Self-directed, innovative, organized, strong personal work ethic.
  • Good communication skills including listening.
  • Ability to engage and “draw out” others.
  • Decision-making ability.
  • Crisis management skills.
  • Professional personal presentation.
  • Ability to teach/train in a variety of settings.

Responsibilities

  • Prepare and instruct Pre ETS sessions to school students with disabilities.
  • Travel to assigned schools to provide Pre ETS training.
  • Utilize virtual support services as warranted to provide services.
  • Prepare accurate and timely documents to meet funding source requirements.
  • Collaborate with Program Coordinator, QDDP, teachers, and other Transition Specialists in scheduling Pre ETS sessions and developing curriculum.
  • Attend IEP and/or PCP meetings as needed.
  • Provide support to individuals served participating in volunteer work, worksite tours, mock interviews, as well as, to individuals involved in paid employment with the Arc’s integrated worksites.
  • Provide individualized support, training, and supervision for individuals served at the worksite or competitive employment which includes: skills training, building natural supports, identifying job accommodations, assistive technology, etc.
  • Perform specific job analysis, task analysis, situational assessments and job matching activities, when required.
  • Work with participants, employers, families, agency personnel, direct support professionals, Work Programs Coordinator, QDDPs, and other appropriate parties to problem solve issues related to training and employment.
  • Serve as a role model for students/persons served.
  • Serve as an advocate for the persons served/students.
  • Serve as an ambassador for the Arc of Madison County.
  • Prepare and complete assessments and satisfaction surveys.
  • Be punctual and in attendance as scheduled.
  • Assist with community experiences, as needed, to include but not limited to, work site tours, volunteer opportunities, mock interviews, Special Olympics, or other community activities.
  • Ensure information for billing of services is accurate.
  • Assist, as needed, with maintaining staffing ratios to ensure individual safety and wellbeing.
  • Provide, as needed, pre employment training to other individuals receiving services by the Arc of Madison County.
  • Attend training, as appropriate, to become knowledgeable of strategies and current “best practices” in the field of supported employment and/or other areas.
  • Travel to worksites and access varying workspace as necessary.
  • Participate in Person Centered Planning in collaboration with Service Coordination as needed.
  • Prepare Assessments as warranted, etc., in accordance with funding source requirements.
  • Assist in developing and delivering opportunities for persons served to achieve their life goals.
  • Assist in maintaining funding source(s) and CARF certifications by ensuring standards and documentation are met.
  • Assist with providing services through Community Waiver Program.
  • Assist individuals with personal care as needed.
  • Lift, carry, or restrain individuals as needed.
  • Other duties as assigned.
