About The Position

This is an unpaid internship position where the intern, with direct supervision, will assist Transition Specialists with their duties. The role involves supporting formerly incarcerated clients in identifying and accessing necessary resources for successful reintegration into the community. The intern will gain experience in case management, client advocacy, and community resource coordination.

Must be eligible to work in the U.S. without requiring sponsorship.

Responsibilities

Administer problem identification sheets/self-assessments for clients who were formerly incarcerated.
Identify clients' needs, including housing, substance abuse treatment, mental health services, medical services, mentoring, family assistance, counseling, employment training, and education.
Coordinate with service providers to establish linkages for identified client needs.
Accurately document information for client case files and complete release of information forms.
Stay informed of community resources and make referrals to community organizations.
Follow up with clients to ensure services were provided and act as an advocate when issues arise.
Provide exceptional customer service and contribute to a positive team environment.
Assist clients in becoming more positive, productive, and prosocial community members.
Participate in team meetings with the Program Services Coordinator to discuss client issues and case reviews.
Attend Community Events as a representative of North Star NRRC.
Perform other duties as assigned.
