About The Position

This is an unpaid internship position where the intern, with direct supervision, will assist with Transition Specialist duties. The role involves supporting formerly incarcerated clients in identifying and accessing necessary services to aid their reintegration into the community. The intern will gain experience in case management, resource coordination, client advocacy, and documentation within a supportive and therapeutic environment.

Requirements

  • Must be eligible to work in the U.S. without requiring sponsorship.
  • Ability to administer problem identification sheets/self-assessments accurately.
  • Skills in identifying client needs across various domains (housing, health, employment, etc.).
  • Ability to coordinate with service providers.
  • Proficiency in accurate documentation.
  • Understanding of release of information procedures.
  • Willingness to stay informed about community resources.
  • Ability to make referrals and follow up with clients.
  • Capacity to act as an advocate.
  • Strong customer service skills.
  • Ability to contribute to a team-focused environment.
  • Willingness to assist clients in community reintegration.
  • Ability to participate in team meetings and case discussions.

Responsibilities

  • Administer problem identification sheets/self-assessments for clients.
  • Identify client needs, including housing, substance abuse treatment, mental health services, medical services, mentoring, family assistance, counseling, employment training, and education.
  • Coordinate with service providers to facilitate client access to identified needs.
  • Accurately document client information for case files.
  • Complete release of information forms for referrals and information sharing.
  • Stay informed about community resources.
  • Make referrals to community organizations for clients.
  • Follow up with clients to ensure services were provided.
  • Act as an advocate for clients when issues arise.
  • Provide exceptional customer service and contribute to a positive team environment.
  • Assist clients in becoming more positive and productive community members.
  • Participate in team meetings and discuss client issues/case reviews.
  • Attend community events as a representative of North Star NRRC.
  • Perform other duties as assigned.
