About The Position

At Rebecca School, we support students ages 3–21 with neurodevelopmental delays through the DIRFloortime® model. As a Transition Coordinator, you will play a vital role in helping students prepare for life beyond school by supporting transition planning, independence, community engagement, and long-term life goals. You will work closely with students, families, and multidisciplinary teams to develop individualized plans that promote growth, self-advocacy, and successful post-school outcomes. Through collaboration, resource coordination, and program development, you will help students build the skills and connections needed to thrive in their communities and future endeavors.

Requirements

Bachelor's degree in Education, Special Education, Social Work, Psychology, Human Services, or a related field preferred
Experience working with adolescents and young adults with diverse learning, developmental, or behavioral needs
Strong communication, collaboration, and relationship-building skills
Knowledge of transition planning, community resources, and person-centered planning practices
Ability to coordinate multiple projects and manage competing priorities
Experience working with families, school teams, and community partners
Strong organizational and documentation skills
Proficiency with Microsoft Office and Google Workspace

Responsibilities

Maintain a transition planning caseload of students ages 14–22 and collaborate closely with students, families, and educational teams
Lead the development of long-term life planning goals and short-term transition objectives
Coordinate the School Independence Program (SIP) and support student participation
Partner with educational supervisors on curriculum planning, community experiences, and the Best Buddies program
Assist with planning and implementing extracurricular opportunities for students and families
Develop and maintain community partnerships and transition-related resources
Support and manage a city-wide resource and referral directory for students and families
Advocate for student independence, community engagement, and successful post-school outcomes
