At Rebecca School, we support students ages 3–21 with neurodevelopmental delays through the DIRFloortime® model. As a Transition Coordinator, you will play a vital role in helping students prepare for life beyond school by supporting transition planning, independence, community engagement, and long-term life goals. You will work closely with students, families, and multidisciplinary teams to develop individualized plans that promote growth, self-advocacy, and successful post-school outcomes. Through collaboration, resource coordination, and program development, you will help students build the skills and connections needed to thrive in their communities and future endeavors.

Requirements

  • Bachelor's degree in Education, Special Education, Social Work, Psychology, Human Services, or a related field preferred
  • Experience working with adolescents and young adults with diverse learning, developmental, or behavioral needs
  • Strong communication, collaboration, and relationship-building skills
  • Knowledge of transition planning, community resources, and person-centered planning practices
  • Ability to coordinate multiple projects and manage competing priorities
  • Experience working with families, school teams, and community partners
  • Strong organizational and documentation skills
  • Proficiency with Microsoft Office and Google Workspace

Responsibilities

  • Maintain a transition planning caseload of students ages 14–22 and collaborate closely with students, families, and educational teams
  • Lead the development of long-term life planning goals and short-term transition objectives
  • Coordinate the School Independence Program (SIP) and support student participation
  • Partner with educational supervisors on curriculum planning, community experiences, and the Best Buddies program
  • Assist with planning and implementing extracurricular opportunities for students and families
  • Develop and maintain community partnerships and transition-related resources
  • Support and manage a city-wide resource and referral directory for students and families
  • Advocate for student independence, community engagement, and successful post-school outcomes
