About The Position

The Transition Community Facilitator supports students with disabilities in both community and classroom settings. Key responsibilities include providing job coaching, creating task analyses, and preparing checklists to aid students in completing job tasks. The role involves implementing instructional components in the classroom under the guidance of a Special Education Teacher, adapting work environments to student skills, and monitoring student performance for progress. The facilitator is also responsible for maintaining accurate records, communicating with students, staff, administrators, and employers, and building positive relationships with employers. Additionally, the position involves providing transportation training, assisting students with accessing transportation, and helping them acquire independent living skills. The facilitator is expected to maintain professional standards, support the integration of assistive technology, and occasionally provide physical assistance to students with special daily living needs.

Requirements

  • Two years successful post high school education
  • Two years’ experience with students with disabilities with at least one year of experience in a position requiring interpersonal interaction skills
  • Knowledge of programming for students with disabilities.
  • Valid driver’s license and ability to use personal vehicle for work.
  • Excellent oral and written communication skills.
  • Skills in human relations and implementation of student or adult programs.
  • Demonstrated computer proficiency with experience in word processing, databases, and spreadsheets.
  • Confirmed ability to work as a team member and demonstrated organizational skills required.

Responsibilities

  • Supports students in the community with job coaching, creates task analysis and prepares checklist to assist students in completing required job tasks.
  • Implement instructional components in the classroom under the guidance of the Special Education Teacher.
  • Works to support the work environment to adapt job requirements to student skills.
  • Monitors and problem solves students’ performance for progress monitoring.
  • Maintains clear, accurate records of student performance and communicates with students, staff, administrators, and employers.
  • Maintains employer standards and builds positive, long-term relationships with employers.
  • Provides transportation training and assists students in accessing transportation.
  • May be requested to transport students using personal vehicle for community outings.
  • Assists students to acquire independent living skills in the classroom and community.
  • Maintains professional standards as a representative of the District.
  • Supports integration of assistive technology for students.
  • Occasionally provides physical assistance to students with special daily living needs.
  • Performs other duties as assigned by supervisors.
