About The Position

The Transition Care Coordinator (TCC) for the AFW2 Program is responsible for ensuring transition services are provided to WII Airmen/Recovering Service Members (RSMs). The program aims to establish a process for integrated continuity of world-class quality care and service delivery, focusing on quality assurance and improved resource allocation. This role involves collaboration across various sectors and with unified common standards of care. The TCC assists Airmen, families, and caregivers in navigating the continuum of care, managing and supporting the RSM's care from recovery and rehabilitation to reintegration into active duty, civilian life, or as a veteran. The TCC works with a multi-disciplinary team to develop and manage the Comprehensive Recovery Plan (CRP).

Requirements

• Prior Air Force experience required.
• Organization and communication skills.
• Interpersonal skills: ability to work with multiple agencies to accomplish project objectives.
• MS Office Proficiency and experience with Database Systems.
• Must be a U.S. Citizen.
• Must be able to pass an extensive background investigation.

• Previous active duty military experience or experience working with Service members or veterans preferred.
• Leadership Experience working with Wounded, Ill or Injured Service Members and/or their families.

Responsibilities

• Provide specialty support for transition, finance, personnel, and other specific issues to support Airmen.
• Oversee the development, implementation, and retain oversight of the Comprehensive Recovery Plan (CRP).
• Consult and collaborate with Multidisciplinary Teams (MDT) throughout the continuum of care.
• Oversee, coordinate, and monitor the non-medical services across the continuum of care as documented in the CRP.
• Assist in rehabilitation and transition back to serving reserve status or civilian life as a veteran through coordination with Military Service Coordinators and VA Liaisons.
• Collect, maintain, and produce data for reporting purposes in accordance with governing program guidance.
