The Transition Coordinator is responsible for processing patient referrals and maintaining client information in a database. This role provides clerical and administrative support to DME Customer Service in day-to-day care management activities and telephonic support for incoming and outgoing calls from patients, providers, Transition Coach and other team members related to the Transition Care program. The coordinator is also responsible for expanding, updating and maintaining the community resource database.
