About The Position

The Transition Care Follow Up Specialist is responsible for contacting patients to schedule outpatient follow-up appointments. This role requires coordination of patient availability with clinic schedules, potentially involving rescheduling appointments. The specialist will also document patient encounters in the EMR, contact clinical staff for order clarification when needed, perform general office duties such as faxing sensitive information, and participate in performance improvement activities. The position requires demonstrating knowledge and skills appropriate for the age of patients served, including understanding their physical and psychological needs and responding effectively.

Requirements

  • A minimum of six months of healthcare experience
  • 1 Year Previous Epic User experience
  • Proficient in medical terminology
  • Able to handle multiple responsibilities/tasks, stress, and high work load volumes
  • Ability to easily adapt to change
  • Strong organizational, written and oral communication skills
  • Creative problem-solving skills
  • Ability to demonstrate persistence in follow-up
  • Superior time management skills, attention to detail, excellent interpersonal skills
  • Ability to work as part of a multidisciplinary team

Responsibilities

  • Contacts patients at bedside and/or telephonically to schedule outpatient follow up appointments.
  • Requires coordination of patient availability and clinic schedules which may require rescheduling of appointments.
  • Escalates when clinical judgement is needed.
  • Documents all patient encounters into EMR accurately.
  • Contacts clinical staff when needed for clarification of order.
  • Performs general office duties including but not limited to faxing required sensitive information to providers to assist clinical staff with patient services.
  • Participates in performance improvement activities.
  • Duties as assigned
