About The Position

The Transition Specialist is responsible for using sound professional judgement and best practices to ensure quality case management services necessary to assist youth who are transitioning out of foster care in achieving their goals. This role involves assessing client needs, obtaining resources, advocating with various systems, assisting youth in crisis, developing individual service plans, and maintaining accurate documentation. The specialist must adhere to all HHS guidelines and policies, demonstrate knowledge of contracts and agency procedures, and participate in program events. Measurable deliverables include meeting federal and state regulatory guidelines, participating in program events, working evenings or weekends as needed, implementing safety protocols, obtaining service approvals, demonstrating knowledge of community resources, maintaining confidentiality, upholding ethical standards, attending trainings, recording services in the Salesforce database, reporting suspected child abuse and neglect, and responding sensitively to the service population's cultural and socio-economic characteristics. The role requires effective written and verbal communication in English, the ability to work in a fast-paced environment, computer literacy, and the capacity to work independently and as part of a team. Travel may also be required.

Requirements

Bachelor’s degree in Health and Human Services, Social Services, Education or Criminal Justice
2 years of experience in case management, job development or crises intervention with youth
Experience in community building, event coordination and public speaking.
Maintain certificates required for the position including but not limited to First Aid and CPR.
Valid Texas Driver’s license with a clear driving record and reliable transportation.
Pass a pre-employment drug screen and random drug screens.
Provide proof of work eligibility status upon request.
Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
Bilingual. Spanish required. English (United States)

Responsibilities

Assess the personal needs of PAL clients and help to obtain resources to meet those needs by providing case management services.
Advocate with human services, health, and court and criminal justice systems to resolve issues facing clients.
Assist youth in crisis by providing de-escalation for situation and developing plan with youth to move out of state crises.
Develop detailed Individual Service Plans for each client and meet with them regularly to assess progress and update the plans.
Maintain timely and accurate file documentation and data entry that meet the requirements of the PAL program.
Follow all HHS guidelines and policies concerning PAL TFSS services.
Demonstrate a working knowledge of all contracts and agency policies and procedures.
Participate/lead PAL program weekly, monthly events and activities.
Implement BCFS HHS safety protocols including evacuating with children and other staff in case of an emergency.
Get approval for services through authorization form (2054) from HHS PAL staff.
Show knowledge of community agencies and resources.
Maintain confidentiality in all areas of the service population and program operations.
Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook including demonstrating respect for agency staff, the service population, and community members and complying with the required dress code, at all times.
Attend all required program trainings as applicable.
Record all services and activities in a complete, concise, and timely manner utilizing program forms and Salesforce database.
Report on any suspected child abuse and neglect to the program director and law enforcement agency.
Respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
Communicate effectively in writing and verbally in English.
Work in a fast-paced environment.
Maintain computer literacy required to meet the responsibilities of the position.
Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
Travel as needed.
