The Transition Specialist is responsible for using sound professional judgement and best practices to ensure quality case management services necessary to assist youth who are transitioning out of foster care in achieving their goals. This role involves assessing client needs, obtaining resources, advocating with various systems, assisting youth in crisis, developing individual service plans, and maintaining accurate documentation. The specialist must adhere to all HHS guidelines and policies, demonstrate knowledge of contracts and agency procedures, and participate in program events. Measurable deliverables include meeting federal and state regulatory guidelines, participating in program events, working evenings or weekends as needed, implementing safety protocols, obtaining service approvals, demonstrating knowledge of community resources, maintaining confidentiality, upholding ethical standards, attending trainings, recording services in the Salesforce database, reporting suspected child abuse and neglect, and responding sensitively to the service population's cultural and socio-economic characteristics. The role requires effective written and verbal communication in English, the ability to work in a fast-paced environment, computer literacy, and the capacity to work independently and as part of a team. Travel may also be required.
