About The Position

Sentara is hiring a RN Transfer Center Bedflow Coordinator for the Fort Norfolk Transfer Center. This position is full time (36 hours per week), day shift. Transfer Coordinators assess, plan, develop, implement, and evaluate transfers for patient flow throughout the system network. They coordinate and collaborate with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfers. They also coordinate and communicate the capacity management plan to nursing units, leadership team, and physicians. The role requires clinical knowledge and critical thinking skills to meet the standards of care required by the patient population. The ideal candidate must have the ability to work in a fast-paced environment with a positive attitude towards all professional contacts.

Requirements

  • Registered Nurse
  • Experience working in ICU, ED or Transfer Center
  • Recent Acute Care
  • Ability to multitask, type with reasonable speed and take accurate notes.

Responsibilities

  • Assesses, plans, develops, implements, and evaluates transfers for patient flow throughout the system network.
  • Coordinates and collaborates with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfers.
  • Coordinates and communicates capacity management plan to nursing units, leadership team and physicians.
  • Possesses clinical knowledge and critical thinking skills to meet standards of care required by the patient population.
  • Ability to work in a fast-paced environment with a positive attitude towards all professional contacts.
