About The Position

Covenant House California (CHC) is a nonprofit organization dedicated to providing sanctuary and support to youth aged 18-24 who are experiencing homelessness. CHC offers a comprehensive range of services aimed at addressing the physical, emotional, educational, vocational, and spiritual needs of young people, with the belief that every young person deserves housing, food, clothing, education, and love. CHC is seeking individuals who share the commitment to ending youth homelessness and building an equitable work culture. Applicants for this position must be at least 25 years old due to the direct care provided to young adults aged 18-24. Requirements

  • A Master's degree in a behavioral science from an accredited graduate school or an approved graduate program.
  • Experience in a public or private child welfare social services setting.
  • Specific experience working with transition-age youth (16-24 years old) is preferred.
  • Experience with youth experiencing homelessness, mental illness, or system involvement.
  • Previous experience with Title 22 regulations is a bonus.
  • Demonstrated leadership and program development experience are preferred.
  • At least one year of administrative or supervisory experience over social work, childcare, or support staff in an agency or community care facility with a licensed capacity of six or more, OR at least two years of employment as a social worker in an agency serving children or in a group residential program for children.
  • Working knowledge of the Housing First model, Trauma Informed Care, principles of harm reduction, and positive youth development.
  • Knowledge of adolescent development, case management, crisis intervention, and community resources.
  • Awareness and sensitivity to the needs of youth experiencing homelessness and human trafficking.
  • Must be a self-starter and team player with the ability to work collaboratively.
  • Excellent written and verbal communication skills, including public speaking ability.
  • Ability to work independently and as part of an interdisciplinary team.
  • Ability to accurately and professionally represent CHC in the community.
  • Communication with young people must reflect a "strength based" and "trauma informed" approach.
  • Good organizational skills and time management.
  • Strong computer skills including word processing, spreadsheets, email, calendar, and database operations.
  • Must have and maintain a valid California Driver’s License, reliable and efficient transportation, and a good driving record.
  • Must be able to lift up to 40 lbs. on occasion.
  • Must be able to bend, sit, and stand for prolonged periods of time and navigate stairs daily.
  • Must be able to work occasionally in outside weather conditions.
  • Must be at least 25 years old.

Responsibilities

  • Evaluate, place, and assess prospective and enrolled eligible young adults in the Transitional Housing Plus Program (THP-Plus).
  • Work with young adults (18-24) who have been emancipated from foster care (including those supervised by Juvenile Probation) to ensure success in securing stable housing, employment, education, health and wellness, and circles of support.
  • Provide supportive services such as transportation assistance, development of daily living skills, counseling, mental health services, and assistance in obtaining vital documents.
  • Adhere to the Mission Statement, Five Principles, and Policies and Procedures of Covenant House California.
  • Conduct initial intake and full intake assessments within 3-7 days, supporting young people in identifying and accessing relevant resources.
  • Develop, implement, and manage Individual Service Plans (ISPs), including Success Plans (Housing Plans) updated every 30 days and Wellness Surveys completed every 30 days.
  • Conduct regular home visits (monthly for offsite, weekly for onsite) to provide case management support and ensure ongoing engagement.
  • Maintain a calendar of youth’s work schedule and appointments in conjunction with the case plan.
  • Develop and maintain knowledge of and connections to community resources, making appropriate referrals within CHC and to external agencies.
  • Engage in positive transition planning with youth, assisting with housing searches, applications, habitability evaluations, and required documentation.
  • Provide mediation with landlords and neighbors, and offer information on landlord/tenant rights and responsibilities.
  • Review and ensure understanding of lease requirements.
  • Obtain, interpret, and correct rental and credit history.
  • Assist in developing household budgets, reducing expenses, and addressing debt or consumer credit issues.
  • Provide information and referrals for employment and essential goods/services.
  • Partner with community organizations to support payment of security deposits, utility assistance, rental assistance, and other housing-related costs.
  • Monitor and document the progress of youth’s case plans, maintaining case management files (HMIS and ETO) for assigned caseload.
  • Document case notes at least once per week, reflecting services provided and participant progress.
  • Monitor program outputs and ensure program outcomes are met, including documentation for external grants.
  • Document pertinent information in Critical Incident Reports, Wellness Surveys, engagement touchpoints, and Follow-Up forms.
  • Prepare and present assigned cases at department Case Review Meetings.
  • Participate in weekly one-on-one check-ins and case management meetings with the supervisor.
  • Facilitate or support group activities with youth on a regular basis.
  • Facilitate group case management meetings, including mediations and crisis intervention as needed.
  • Be punctual and present for assigned shifts and all scheduled meetings.
  • Attend and complete all required mandatory trainings.
  • Adhere to all Covenant House California (CHC) policies and procedures.
  • Perform duties as assigned by the Director of Community Housing and Alumni Support, including ensuring the cleanliness and safety of the living environment, providing crisis intervention, and offering backup support for other direct care positions.
