About The Position
This position involves working with an interdisciplinary team to achieve treatment goals. The therapist will utilize a variety of factors to evaluate patients' biopsychosocial needs, develop a treatment plan, and create a comprehensive termination of treatment/discharge plan. Accurate documentation of objective data regarding assessments and interventions is also required.
Requirements
- Must have hospice experience - at least 1 year.
- Master's Degree in Social Work (MSW) or Marriage and Family Therapy (MFT) from an accredited school.
- MSW license
