In this role, you will provide high-quality counseling services consistent with professional standards, best practices and Concord’s policies to older adults (60 and over), focusing on supporting their independence and promoting good mental health.

Requirements

  • Master’s degree in social work with independent licensure: LISW/LPCC/LMFT required
  • Ability to maintain thorough, timely, well documented treatment plans, case notes and intake summaries, illustrating caseload measurable treatment progress in computerized medical records system.
  • Must have working knowledge of mental illness, psychiatric symptoms, and their impact on functioning and behavior working with older adults and understanding their unique challenges.
  • Ability to pass a pre-employment background check and drug screen.

Nice To Haves

  • Enrolled on Medicare, Medicare Advantage, and/or private insurance panels is preferred.

Responsibilities

  • Deliver individual therapy to clients with mental health diagnoses and/or substance use disorders
  • Conduct comprehensive assessments and develop individualized treatment plans
  • Apply clinical expertise within a multidisciplinary care model
  • Collaborate with providers and others as needed to support client success
  • Assess risk and respond to crisis situations during office hours
  • Provide services in-person, via telehealth, and in the client’s home, based on client needs
  • Maintain accurate, timely clinical documentation in an electronic medical record system
