Requirements

  • High School Diploma/GED
  • 4 years within mental health setting.

  • Bachelor’s Degree in Human Services.

Responsibilities

  • Provides direct services, initiates, and carries out individual client-related treatment plans and liaison services with the inpatient and outpatient programs and other community services providers.
  • Prepares for a client's discharge from the program by developing a client-specific discharge plan upon program entry and makes community referrals.
  • Provides direct instruction of activities of daily living in accordance with program, county, and state regulations.
  • Reports all maintenance issues to leadership or designee according to policy.
  • Maintains program building and client apartments according to set policies and regulations.
  • Participates in program performance improvement activities.
  • Designs and implements individual treatment plans for clientele.
  • Ensures clientele is receiving appropriate financial resources and coordinates same.
  • Assists program nurse in client medication observation and documentation.
