Optum Home & Community Care is hiring a Behavioral Health Advanced Practice Clinician (PMHNP) for its Care at Home program, a collaborative, population health–focused care delivery model serving Dual Eligible Special Needs Plan (DSNP) members with significant medical, behavioral, and social complexity, including serious mental illness (SMI) and substance use disorders (SUD). The PMHNP will partner with an interdisciplinary care team to provide evidence-based behavioral health care in a home-based and population health model, supporting both direct patient care and consultation. Candidates who live in Wisconsin or Iowa and hold the required licensure may work remotely. The role includes telehealth psychiatric evaluations, diagnosis and treatment, medication management, bridge stabilization, and follow-up for high-risk patients. The PMHNP will also consult with interdisciplinary teams, assist with care planning and triage, coordinate with community and specialty providers, and support continuity of care. The position contributes to team education, case conferences, quality improvement, and accurate documentation in the EMR. Compensation generally ranges from $109,500 to $164,000 in total cash compensation, including base pay and bonus. Compensation may vary based on factors such as local labor markets, education, and work experience, and may increase over time based on productivity and performance. Benefits include a comprehensive benefits package, incentive and recognition programs, an equity stock purchase plan, and 401(k) contribution, subject to eligibility requirements. Remote employees must comply with UnitedHealth Group’s Telecommuter Policy. Candidates must pass a drug test before employment begins.
