Overview Behavioral Health Nurse Practitioner (NP) reports to the Lead Behavioral Health Nurse Practitioner, with accountability for providing direct patient care to manage behavioral health as part of a multidisciplinary clinical team.

Requirements

  • Master’s degree or higher as a BHNP or equivalent degree with a record of strong academic achievement.
  • NP license in the state of practice.
  • Board Certification as BHNP, additional certification as an FNP is a plus.
  • At least 1 year of behavioral health experience.
  • Ideal candidates will have 3+ years of relevant healthcare experience in the healthcare industry.
  • Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to Grace at Home's team, investors, partners and other stakeholders.
  • Proficiency in all Microsoft Office applications.
  • Ability to build and effectively manage relationships with business leaders and external constituents.
  • Good judgement/ impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding and entrepreneurial Company.

Nice To Haves

  • additional certification as an FNP is a plus.

Responsibilities

  • Conduct psychiatric evaluations, assess mental health status, and diagnose mental health conditions.
  • Develop and manage individualized treatment plans based on evidence-based practices.
  • Prescribe, monitor, and adjust psychotropic medications while ensuring patient education on their use.
  • Provide psychotherapy and therapeutic interventions to individuals or groups.
  • Collaborate with other healthcare providers to coordinate comprehensive care.
  • Maintain accurate patient records and document progress in line with regulatory standards.
  • Educate patients and families on mental health, treatment options, and coping strategies.
  • Ensure that nursing capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of Grace at Home.
  • Provide subject matter expertise in the nursing solutions, including determining and recommended approaches for behavioral health/ solution deployment and performance evaluation.
