About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is seeking a Telephone Triage RN. This innovative program serves individuals 55 years or older who are living with chronic illness or disabilities and require coordinated medical care to maintain independence. The healthcare teams provide a hands-on approach, coordinating medical, social, and home care services. This is a Full-time position with a typical schedule of Monday - Friday, 8:00 AM - 4:30 PM. The Triage Registered Nurse (RN) is responsible for providing nursing services to assigned patients. RNs provide direct patient care, collaborate with Interdisciplinary Team (IDT) members, and other healthcare providers. Patient care is delivered in a manner that promotes an interdisciplinary team approach, respects patient and family goals, and meets Medicare/Medicaid conditions of participation and accreditation requirements. The RN practices in alignment with the organization’s mission, values, and clinical philosophy. Upon request, participates in all areas of clinical services.

Requirements

  • Current RN license in the state of California
  • Minimum of one year RN experience – PACE (Program of All-Inclusive Care for the Elderly), clinic, hospice, or home health experience preferred
  • Previous experience as a telephone triage nurse preferred
  • Must be able to clear fingerprints through DOJ and FBI in addition to a drug screen clearance.

Responsibilities

  • Comprehensively assesses and anticipates all aspects of the patient’s clinical nursing needs, pain, and other symptoms and, in collaboration with the primary care physician, develops an individualized plan of care PRN.
  • Incorporates participant/ caregiver preferences and goals within the plan of care.
  • Supports and respects the participant and/or caregivers' unique diversity, including spiritual and cultural beliefs.
  • Completes documentation accurately and completely as per agency standards and in compliance with regulatory standards.
  • Provides patient and family education as appropriate; areas of education include, but are not limited to goals of care, personal care, medications, procedures, safety, diet and nutrition, disease process, death, and dying.
  • Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members.
  • Works collaboratively with volunteers and provides ongoing support and guidance.
  • Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely, open manner; engages collaboratively in conflict resolution processes as needed.
  • Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external.
  • Participates in mandatory educational activities and required meetings.
  • Is competent in using agency information and technology systems.
  • Performs in a supportive role to the IDT – Case Managers, answering calls coming into the agency with response to the telephone call by delegating tasks.

  • Travel to multiple work locations
  • All other duties as assigned.
