This full-time position is responsible for providing patient/family centered care as a partner and advisor. The role assists and supports patients and families in optimally managing their healthcare, respecting their culture, values, individual needs, health goals, and treatment preferences. The nurse will apply critical reasoning and clinical judgment to expedite appropriate care and treatment, assessing patients through telecommunication technologies such as telephone, fax, email, internet, or patient portal. Responsibilities include ensuring proper scheduling and follow-up on test results for providers, using active listening skills, approved protocols, and evidence-based practice to explain test results and medications. The role also involves participating in professional development, maintaining competencies, identifying learning needs, and pursuing lifelong learning to update clinical and professional knowledge. Patient safety and quality of care are paramount, achieved through appropriate nursing interventions, health education, patient advocacy, coordination of services, and evaluation of patient outcomes. The nurse will maintain knowledge of care teams for coordination, self-management, population management, and communication skills, adapting communication style to diverse patient backgrounds. Exceptional assessment, critical thinking, and customer service skills are essential.
