This role provides patient/family-centered care, acting as a partner and advisor to assist and support patients and families in optimally managing their healthcare. The nurse will apply critical reasoning and clinical judgment to expedite appropriate care and treatment, assessing patients through telecommunication technologies such as telephone, fax, email, internet, or patient portal. Responsibilities include ensuring proper scheduling and follow-up on test results for providers, using active listening skills, approved protocols, and evidence-based practice to explain test results and medications. The nurse will adhere to the American Nurses Association Code of Ethics, participate in their own professional development, and demonstrate the ability to function in a professional setting through active participation in a professional practice model, including shared governance, adherence to standards of care, peer review, and professional development. The role focuses on patient safety and quality of nursing care by applying appropriate nursing interventions, coordinating nursing and other health services, assisting patients in navigating the healthcare system, and evaluating patient outcomes. Maintaining knowledge of care teams for coordination of patient care, self-management, population management, and communication skills is essential. The nurse will adapt their communication style to meet the needs of patients from diverse backgrounds and demonstrate exceptional assessment, critical thinking, and customer service skills. Performs other related duties incidental to the work described herein.
