About The Position

Wider Circle is a mission-driven healthcare organization focused on improving health outcomes for older adults and complex populations by addressing social and clinical barriers to care. They partner with health plans and local organizations to help members navigate healthcare, stay engaged, and live healthier lives. A new clinical pilot is being launched that integrates physicians, care navigators, and community support teams for a coordinated and human approach to care. This role involves conducting initial virtual visits with members to assess their medical needs, barriers to care, and health goals, setting the direction for their care journey. Ongoing support is provided by a care team, allowing the physician to focus on clinical guidance. This position is ideal for physicians interested in innovative care delivery, telehealth, and population health models.

Requirements

MD or DO with an active, unrestricted Texas license
Board certified or board eligible (Family Medicine, Internal Medicine, or related primary care specialty preferred)
Comfort working with Medicare, Medicaid, or complex populations
Strong communication skills and ability to build trust quickly in a virtual setting
Interest in care coordination, health navigation, or whole-person care models
Comfort working in a startup-like, evolving program environment

Telehealth experience preferred

Responsibilities

Conduct initial telehealth visits with enrolled members
Assess medical needs, care gaps, and barriers to accessing care
Establish a clear clinical plan and care priorities
Collaborate with care team members to ensure a smooth handoff and continuity of care
Provide clinical insight to help refine program workflows during the pilot phase
