About The Position
Our Telehealth LPNs coordinate the Telehealth program for Catholic Home Care to insure the optimal utilization of the technology to enhance patient care and clinical excellence. Adhering to Catholic Home Care department policies and procedures and TJC/regulatory agency requirements. Demonstrates and promotes service excellence at all times.
Requirements
- 2-3 years recent hospital/home care experience
- Strong clinical skills
- Strong communication and interpersonal skills
- Computer Skills
- Currently Licensed by New York State as a Licensed Practical Nurse (LPN)
Nice To Haves
- Acute care telemetry or triage experience is preferred
