About The Position

Connected Healthcare Partners (CHP) is a growing mobile acute care organization providing high-quality, provider-led care directly to patients in the Greater Houston area. Our innovative care model combines in-home and telehealth services to deliver timely, coordinated, and clinically excellent care. We are seeking an experienced Telehealth Nurse Practitioner (NP) to join our Mobile Acute Care Program. This fully remote role provides direct virtual patient care while also supporting our mobile field clinicians through timely EHR inbox management, clinical coordination, and transitions of care. The ideal candidate is a confident, clinically sound Nurse Practitioner who thrives in a fast-paced environment, demonstrates excellent independent judgment, and is comfortable managing acute and urgent patient needs through telehealth.

Requirements

  • Active, unrestricted Nurse Practitioner license in good standing.
  • Family Nurse Practitioner (FNP) certification required.
  • Current national board certification as an FNP.
  • Minimum of 2 years of clinical Nurse Practitioner experience in one or more of the following: Urgent Care, Emergency Medicine, Acute Care, Telehealth.
  • Strong independent clinical judgment and the ability to make sound clinical decisions in a telehealth environment.
  • Comfortable assessing and managing patients when traditional in-person physical examination findings are not available.
  • Experience managing high-volume EHR inbox workflows and clinical communications.
  • Excellent written and verbal communication skills.
  • Strong organizational skills and ability to prioritize multiple clinical tasks effectively.
  • Ability to work independently while collaborating effectively with a multidisciplinary care team.
  • Reliable high-speed internet connection.
  • Private, secure, and HIPAA-compliant workspace suitable for telehealth practice.

Nice To Haves

  • Emergency Nurse Practitioner Certification (ENP-C) in addition to FNP certification.
  • Active Texas Nurse Practitioner license.
  • Multi-state Nurse Practitioner licensure.
  • Experience with Athena EHR or similar electronic health record systems.
  • Experience providing care in mobile medicine, home-based care, hospital-at-home, urgent care, emergency medicine, or transitional care settings.
  • Experience with Transitional Care Management (TCM).
  • Experience collaborating with field-based clinicians and multidisciplinary care teams.

Responsibilities

  • Conduct telehealth visits with patients using a secure, HIPAA-compliant video and audio platform, including acute, urgent, and follow-up visits.
  • Obtain comprehensive virtual patient histories and perform appropriate virtual assessments.
  • Evaluate symptoms, establish differential diagnoses, and develop appropriate diagnostic and treatment plans.
  • Conduct Transitional Care Management (TCM) visits for patients following discharge from hospitals, skilled nursing facilities, rehabilitation facilities, and other care settings.
  • Order, review, and interpret laboratory testing, imaging, and other diagnostic studies as clinically indicated.
  • Prescribe medications within the NP's scope of practice, state licensure, and applicable CHP clinical protocols.
  • Manage the Mobile Acute Care team's EHR inbox, including clinical messages, patient updates, diagnostic results, refill requests, and other incoming tasks.
  • Respond to clinical inquiries and EHR tasks in a timely manner consistent with CHP standards and patient care priorities.
  • Collaborate closely with mobile field clinicians regarding patient escalations, changes in clinical status, care plan updates, and transitions of care.
  • Identify patients who may require an in-person evaluation and coordinate appropriate escalation to the Mobile Acute Care team.
  • Maintain timely, accurate, and complete documentation of all patient encounters and clinical activities in Athena EHR.
  • Ensure documentation meets clinical, billing, regulatory, and compliance requirements.
  • Participate in case reviews, quality improvement initiatives, clinical meetings, and protocol development as the program grows.
  • Maintain a professional, private, and HIPAA-compliant workspace suitable for remote clinical practice.
  • Follow CHP policies, clinical protocols, quality standards, and applicable federal and state regulations.
