About The Position

This is a grant-funded (or term-limited) position. Continued employment is contingent upon the availability of grant funding. The Telehealth Clinical Support Specialist is a grant-funded position that reports to the Collaborative Care Clinical Manager. This position serves as a Clinician Support Specialist for a telehealth-enabled collaborative care program, supporting patients and clinicians in rural primary care sites. The Clinical Support Specialist supports patient engagement in the CoCM program, regularly follows-up with patients between sessions, provides and links patients and families to referrals as needed. Documents appropriate encounter time for CoCM and BHI services. Assist is scheduling and rescheduling as needed. Prepare monthly clinical billing documentation for assigned providers. Assist with response to urgent care alerts. Occasionally administers monthly screener/questionnaires as needed. Proactively communicates patient needs and treatment concerns to the BHCM. The Clinical Support Specialist will also engage in caseload review with assigned BHCM for update to date list of lost to follow-up patients needing outreach and other duties as assigned.

Requirements

  • High School Diploma required
  • 3-5 years of healthcare experience, discharge planning, patient coordination and scheduling
  • Critical thinking skills
  • Strong interpersonal skills
  • Excellent organizational and time management abilities
  • Able to work effectively within a multidisciplinary team
  • Skilled with patient interaction, problem solving, navigation, and listening

Nice To Haves

  • Bachelor’s degree from an accredited college/university in social work, psychology, counseling, or social welfare program preferred
