As a Telehealth Case Manager, you will provide support, educating clients and caregivers in areas including health maintenance and preventative measures, assisting in healthcare management navigation, and accessing community support. Requirements

  • Valid driver’s license and reliable transportation required.
  • Bachelor’s degree in human services, in public health or another related field required.
  • A minimum of two years’ recent experience in the direct care of elders or adults with disabilities required.
  • Comprehensive knowledge of case management principles, healthcare management and reimbursement.
  • Strong communication, documentation and organizational skills required.
  • Must be solution oriented and able to multitask.
  • CPR Certification/Willingness to obtain one.

Nice To Haves

  • Fluency in both English and second language strongly preferred.
  • Case management, behavioral health, social services and/or community health worker experience preferred.

Responsibilities

  • Complete all visit types in accordance with Mass Health regulations
  • Complete documentation in accordance with regulations and Vitra polices at the end of each client and or caregiver interaction
  • Navigate clients and caregivers through other healthcare resources and community services.
  • Participate in multidisciplinary care plan development, review and discharge planning.
  • Address concerns regarding environment and client status changes and escalates it accordingly.
  • Participate in multidisciplinary team meetings and work collaboratively to address client and caregiver needs.
  • Complete all required documentation per regulation and communicate professionally with client, Primary Care Physicians and other healthcare providers when necessary.
  • Facilitates Physicals and any other client appointments with their PCPs
  • Act as a steward of quality and compliance by learning policies in procedures in accordance with regulations and Vitra’s quality standards
  • Leads care and emergency plan education for clients and caregivers
  • Participates in performance improvement initiatives such as audits, peer teaching, etc.
  • Perform other duties as assigned.
