About The Position

Seniority Healthcare is seeking a compassionate healthcare professional to join their team as a COMPASS Nurse. COMPASS stands for Collaborative Outpatient Management for Palliative and Aging Support Services. This role involves providing in-home nursing and case management services to geriatric patients, aiming to make a significant impact on their lives. This is a full-time position, Monday through Friday, with no weekend or on-call responsibilities.

Requirements

• Minimum of 2 years of experience in a similar role, working with geriatric populations.
• LPN or RN license.
• Proficiency in EHR systems.
• Strong communication skills and ability to work effectively in a team environment.
• LPN or RN (Required)

• In-depth knowledge of health care services and medical terminology preferred.

Responsibilities

• Communicate effectively with patients, families, providers, and community staff regarding all aspects of patient care, including updates on treatment plans, addressing concerns, and ensuring informed decision-making.
• Manage and coordinate patient care, including documenting and assisting with orders for testing, medications, and referrals.
• Upload and document all health information into the patient's chart accurately and timely, maintaining records of interactions, treatments, and outcomes.
• Obtain vital signs and assist providers during patient visits, providing hands-on care and support to patients and their families.
• Conduct medication reconciliation and maintain a professional relationship with the PCP, ensuring safe and effective medication management.
• Coordinate referrals, therapy orders, and telemedicine appointments to ensure patients receive necessary services promptly.
• Alert providers of acute issues by monitoring patient conditions and reporting any changes or concerns.
• Check for patient updates and communicate with case management departments to ensure continuity of care.
• Maintain HIPAA compliance and communicate effectively with other members of the care team.
