About The Position

The Team Manager Triage is responsible for directing all patient care services for the team, including volunteer and bereavement services, ensuring they are integrated into the interdisciplinary plan of care. This role involves reviewing patient admissions, discharges, and ongoing care to ensure eligibility, appropriateness, and responsiveness to patient/family needs. The Team Manager also manages the team schedule for 24/7 coverage, facilitates team meetings, and participates in on-call rotations. Additionally, the position involves staff supervision and management, quality improvement, regulatory compliance, customer service, cost containment, and professional development.

Requirements

  • Reliable transportation with appropriate license and insurance coverage for driver and passengers.
  • Private telephone in home.
  • Knowledge of the principles and practices of primary medical and nursing care.
  • Working knowledge of oncologic nursing and palliative care.
  • Basic understanding of interpersonal dynamics within an interdisciplinary health care team.
  • Knowledge of the process of dying and bereavement.
  • Working knowledge of VITAS information system (Vx) and ability to utilize management reports.
  • Two years successful supervisory experience or equivalent in a health care organization.
  • Current and valid R.N. License to practice in the state where the VITAS program is located.

Responsibilities

  • Directs all patient care services for the team, including volunteer and bereavement, ensuring integration into the interdisciplinary plan of care.
  • Reviews pending patient admissions, new admissions (assessments and history), and 'alive' discharges to ensure eligibility and appropriateness criteria are met.
  • Collaborates with team and attending physicians regarding palliative care for pain and symptom management.
  • Ensures complete and accurate data and documentation for timely recertification decisions.
  • Reviews imminent 'alive' discharges to ensure effective discharge planning.
  • Monitors the type and level of team services (staffing, medications, HME, supplies, level of care changes) to ensure appropriateness and responsiveness to patient/family needs.
  • Manages the team schedule and ensures coordination of services 24/7.
  • Facilitates team meetings, ensuring interdisciplinary input into the plan of care updates.
  • Participates in on-call rotation.
  • Interviews, selects, trains, supervises, evaluates, and dismisses team staff in conjunction with the Patient Care Administrator.
  • Ensures staff competence and performance through field visits, documentation evaluation, and team-based services.
  • Acts as a resource and mentor for staff on clinical issues, documentation, problem-solving, and customer service.
  • Oversees staff and volunteer schedules, scheduling, and territory assignments for equitable workload distribution and productivity.
  • Reviews and approves payroll, ensuring accurate time, mileage, and expense documentation.
  • Monitors clinical records and team activities for accuracy, completeness, and compliance with VITAS policy and regulatory bodies.
  • Develops and implements performance improvement activities to address service issues.
  • Ensures team completes patient/family satisfaction surveys and implements Quality Improvement measures.
  • Assists with surveys and requests from regulatory bodies and intermediaries.
  • Performs substantive chart reviews to ensure quality care delivery.
  • Participates in Outcomes Management and annual program review.
  • Performs utilization review of continuous care and inpatient levels of care.
  • Ensures problems/grievances/service failures are addressed and resolved promptly.
  • Analyzes customer service issues to identify causes and works with staff to improve performance.
  • Speaks with patients/families and physicians considering revocation to seek satisfactory solutions.
  • Regularly visits LTC and Contract Bed facilities to ensure care plan integration, customer satisfaction, and discuss criteria.
  • Participates in providing inservices to customers.
  • Participates in professional, voluntary, or community service organizations.
  • Verifies/approves admitting IPOC diagnosis, treatments, staffing, and supplies.
  • Monitors resource utilization for cost-effective service delivery.
  • Controls and is accountable for productivity, labor, and patient care costs relative to budget.
  • Approves all bills/invoices related to patient care services.
  • Attends inservices, educational seminars, and workshops.
  • Develops and achieves professional growth goals and objectives.
  • Participates as a mentor of newly hired Team Managers.
