About The Position

A Team Lead, or Team Leader, is a professional who provides guidance, direction, and leadership to a team of employees, ensuring they achieve organizational goals and objectives. They are responsible for overseeing daily operations of a site-specific home(s), delegating tasks, setting deadlines, and fostering a positive and productive work environment. Team leads also play a crucial role in communication, problem-solving, and performance management within the team.

  • Valid driver’s license
  • CPR/FA certification
  • 2 years' experience in a supervisory role required.
  • Knowledge and ability to use basic computers and technology.

Nice To Haves

  • 1-2 years’ experience working with individuals with disabilities and/or related field preferred but not required.

Responsibilities

  • Inspire and motivate their team members to achieve performance goals and maintain a positive work atmosphere.
  • Establish clear objectives and deadlines for the team and individual members.
  • Identify and resolve issues that may arise within the team or with projects.
  • Report on team performance and progress to higher management.
  • Cultivate positive relationships with guardians as well as the greater community.
  • Track team and individual performance, providing feedback and support to improve outcomes.
  • Provide transportation and supervision for individuals as required to meet employment, recreational and leisure activities.
  • Manage resources allocated to the team, such as budget, tools, or personnel.
  • Build positive relationships and foster collaboration within the team.
  • Perform inspections of vans and ensures that all training for the safe transport of residents is followed including performing tie downs, seat belt usage.
  • Perform housekeeping responsibilities to include but not limited to, kitchens, bathrooms, windows, bedrooms, and floors.
  • Maintain all standards when preparing meals and managing diets. Follows guidelines for safe handling of food as well as ensures adequate supplies. Ensures meals are prepared according to Individual Healthcare plans.
  • Provide personal care support to individuals which includes changing, dressing, and other ADL’s.
  • Complete all documentation and notes as required using the Therap system and other electronic systems.
  • Ensure that goals are being completed according to each individual’s ISP.
  • Conduct evacuation drills and records results.
  • Assist in the planning and facilitation of person-centered outings.
  • Complete behavioral tracking logs as required.
  • Follow all agency, State and Federal laws and policies. Use Policy Stat as needed to access policies.
  • Complete all required training. Use Relias to either complete training or register for courses.
  • Manage medical emergencies according to the health service plan. Attend doctors’ appointments, transports residents to medical appointments and emergency room as required and requested.
  • Identify and report maintenance issues to supervisor or through Net Facilities if available.
  • Report to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observe policies/procedures governing requests for annual, sick, or other leave time.
  • Complete Medication Administration and Proxy training. Administer meds to residents and follow all policies related to MAR entries.
  • Track vital signs and other health related information for residents.
  • Perform all other duties as assigned.
