About The Position

Heritage Behavioral Health Center has served individuals and families across central Illinois for 70 years. Our Supportive Housing Program reflects our commitment to helping adults with mental health and/or co-occurring substance use diagnoses live as independently and healthfully as possible. Guided by the principles of Community Supportive Housing, Permanent Supportive Housing (PSH), and Housing First, we meet clients where they are — fostering overall well-being, community connection, and lasting stability. The Team Lead of Supportive Housing is a working leader responsible for the daily operations, quality of care, staff supervision, and outcome accountability of Heritage's Supportive Housing Program in Decatur, Illinois. This position provides on-site field support, direct coaching, training, and clinical supervision to Supportive Housing Specialists. The Team Lead ensures the delivery of recovery-oriented, trauma-informed services consistent with the philosophies of Community Supportive Housing, Permanent Supportive Housing, and Housing First to adults with mental health diagnoses and/or co-occurring mental health and substance use diagnoses.

Requirements

Bachelor's degree or higher in social work, psychology, counseling, nursing, or a related human services field
Minimum of two (2) years of experience providing mental health or supportive housing services to adults with serious mental illness
Minimum of one (1) year of supervisory, lead, or program coordination experience in a behavioral health or social services setting
Knowledge of Housing First, Permanent Supportive Housing, and Community Supportive Housing models
Valid Illinois driver's license and reliable transportation

Meets the definition of a Qualified Mental Health Professional (QMHP) as defined by the Illinois Department of Human Services preferred
Licensed clinical credential (LCSW, LCPC, LPC, or equivalent) in Illinois
Experience with Medicaid-funded behavioral health programs and documentation systems
Training in WRAP and/or SAMHSA evidence-based practices
Experience working with individuals experiencing housing instability, co-occurring disorders, or chronic health conditions
Experience in activities, recreation, and skill development

Responsibilities

Serve as an active working leader by maintaining a consistent on-site presence across Decatur supportive housing locations and modeling best practices in engagement
Provide direct field supervision, real-time coaching, and professional development to Supportive Housing Specialists and Life Skills Specialists through observation, feedback, and mentoring
Conduct formal staff supervision meetings, performance evaluations, and corrective action processes in accordance with Heritage policies
Oversee staff scheduling to maintain on-site coverage requirements across all supportive housing sites
Lead hiring, onboarding, and orientation of Supportive Housing Specialists and Life Skills Specialists
Review and authorize activities and supports from a clinical perspective, ensuring plans are recovery-oriented, trauma-informed, and aligned with each client's strengths, needs, abilities, and preferences
Monitor program outcomes including housing stability, engagement, and community integration — reporting regularly to senior leadership
Ensure compliance with Illinois DHS Division of Behavioral Health and Illinois Department of Healthcare and Family Services (HFS) standards, billing requirements, and documentation expectations
Maintain working knowledge of and ensure staff adherence to evidence-based practices including WRAP, Illness Management and Recovery, SAMHSA's Integrated Treatment for Co-Occurring Disorders, Supported Employment (IPS), and motivational interviewing
Coordinate access to 24-hour crisis services and ensure trained Mental Health Professionals (MHP) and Qualified Mental Health Professionals (QMHP) remain accessible to clients at all times
Develop and maintain collaborative linkage agreements and working relationships with community partners (e.g., Crossing Healthcare, SIU Healthcare, Macon County Health Department, DRS, Catholic Charities, Salvation Army, DOVE)
Facilitate or oversee monthly client meetings, quarterly satisfaction surveys, and Peer Advisory Council programming
Manage program budget, oversee Medicaid documentation, and ensure appropriate billing and reporting to DHS and HFS funders
Identify and address service gaps, safety concerns, and client crises with swift, person-centered responses
Represent Heritage's Supportive Housing Program in community coalitions, stakeholder meetings, and with housing partners and landlords
Promote a culture of dignity, equity, respect, and recovery within the program and among all staff
