Requirements

• Must be at least 18 years of age.
• High school diploma (or equivalent).

Nice To Haves

• 1-2 years paid experience preferred within behavioral/mental health field.
• Spanish speaking/bilingual preferred.

Responsibilities

• Perform general outreach, engagement, and follow-up with members.
• Coordinate services/appointments.
• Engage clients in health promotion activities.
• Share information with Care Manager on the member's circumstances.
• Make and track referrals.
• Participate in case conferences.
• Support the care manager in assessing and addressing unmet health-related resource needs.
