About The Position

Hope Services, LLC provides evidence-based mental health services to children, adolescents, and families. This is a full-time, direct care position supporting members and the care manager. The Extender performs outreach and follow-up, coordinates services, supports health promotion, shares information about members’ circumstances, tracks referrals, participates in case conferences, and helps identify and address unmet health-related resource needs. The position is for Cumberland County, North Carolina. Spanish proficiency is preferred.

Requirements

• Must be at least 18 years of age.
• Must have a high school diploma or equivalent.
• Lived experience with I/DD or TBI and knowledge of the NC Medicaid system.
• Certified Peer Support Specialist with behavioral health lived experience.
• Parent or guardian with at least two years of direct care experience; experience caring for one’s own child does not qualify.

Nice To Haves

• Spanish language proficiency; Spanish-speaking or bilingual ability preferred.

Responsibilities

• Perform general outreach, engagement, and follow-up with members.
• Coordinate services and appointments.
• Engage clients in health promotion activities.
• Share information with the Care Manager about the member’s circumstances.
• Make and track referrals.
• Participate in case conferences.
• Support the care manager in assessing and addressing unmet health-related resource needs.
