About The Position

The Targeted Case Manager acts as a member of a multidisciplinary team to assess and coordinate patients’ care among multiple service providers during inpatient and outpatient treatment. Our company operates Residential Treatment Programs for individuals with Substance Abuse Disorder. Our mission is to not only restore sobriety but to transform our patients' worlds from a state of everlasting darkness to vibrant technicolor. We believe that treatment is local, individualized, holistic, and relational. We currently have a presence in several states and are rapidly expanding nationwide.

Requirements

  • Bachelor’s Degree required
  • Valid, unrestricted driver’s license
  • Clean driving record for at least 3 years
  • Minimum 1 year experience working directly with adults or adolescents in a human service setting
  • Commitment to a drug-free lifestyle and responsible alcohol use
  • Must meet all applicable State Regulations
  • Submission of Official College Transcripts from an Accredited University

Nice To Haves

  • Previous Targeted Case Management experience or supervision preferred
  • Targeted Case Management (TCM) certification preferred, or must complete department-approved targeted case management training within 6 months of employment
  • Personal passion or interest in substance abuse issues strongly preferred

Responsibilities

  • Assess patient biopsychosocial stressors (e.g., housing, financial, legal, medical, employment) and develop strategies to address these needs during and after treatment
  • Facilitate multiple care aspects including case coordination, information sharing, aftercare planning, and court-related issues
  • Maintain positive working relationships with support networks, government and community resources, employers, and referral sources
  • Complete all necessary documentation related to FMLA and Short-Term Disability
  • Maintain patient records by reviewing and completing case notes
  • Prepare patient discharges by reviewing and coordinating community services needed to maintain sobriety outside of a structured setting
  • Participate in weekly multidisciplinary treatment team meetings to assess patient needs and discharge planning
  • Engage in daily census management, coordinating discharge schedules and communicating with the treatment and administrative teams
  • Continuously maintain professional licensure through required Continuing Education
  • Transport patients in company vehicle to and from Intensive Outpatient Programs (IOP)
