About The Position

The Targeted Case Manager provides Rule 79 Case Management services to eligible persons primarily referred directly from Hennepin County or self-referral, who have been diagnosed with a serious and persistent mental illness. TCM services assist with access to medical, social, and other services. Targeted Case Managers ensure continuity of care coordination and monitoring of services, discharge planning, housing support and intensive goal planning through the development of the Individual Community Support Plan. The Targeted Case Manager would utilize a person-centered approach to work in partnership with persons served with the goal of improved functioning through acquisition of necessary supports, skill development and continuity of services. Here at SpringPath we provide services to people-served who are experiencing significant mental health/psychiatric symptoms, who are demonstrating significant functional impairments, are often experiencing complex challenges with substance use, poverty, being unhoused and often intersect with the criminal justice system. This individual will works within an interdisciplinary team intended to provide intensive, frequent, wrap-around services to individuals who have a diagnosis of serious and persistent mental illness. This Generalist Case Manager/PRP is assigned to persons served individualized treatment teams and work along-side other specialists in carrying out rehabilitative interventions. As part of the person(s) served individualized treatment team, the Case Manager/PRP is responsible for conducting assessments that monitor the person(s) served’s goals and functioning on an ongoing basis. The Case Manager/PRP typically sees approximately 16 to 20 persons served within 1 working week and is expected to communicate interactions with the person served at daily team meetings and documenting these interventions in the Electronic Health Record.

Requirements

  • Bachelor's degree in one of the behavioral sciences or related fields from an accredited college or university, and/or at least 2000 hours of supervised experience in the delivery of services to persons with serious and persistent mental illness; or holds a graduate degree in one of the behavioral sciences or related fields.
  • Individuals with 2,000 hours of experience in the delivery of mental health services to adults
  • Must have fine vision, sustained vision, and peripheral vision.
  • Must possess sufficient eye/hand coordination to operate office, adaptive, and household equipment
  • Must be able to bend, stoop, push, pull reach, sit, and walk for periods of time.
  • Must be able to utilize proper body mechanics while lifting up to 75 pounds.
