Targeted Case Manager - Garden City
About The Position
The TCM Specialist works in Children's Community Based Services of Compass BH. The primary function of the TCM Specialist is to provide medically necessary services as set forth in the consumers treatment plan/ plan of care. The TCM will coordinate, monitor and complete all waiver paperwork for CBS waiver consumers. This position will be responsible for helping individuals access needed medical, social, educational, and other services. The TCM Specialist will be responsible for developing wraparound teams and plans of care, monitoring treatment plans/plans of care and progress notes to assure the plan is working and make changes as needed.
Requirements
- A BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education.
- Demonstrated interpersonal skills.
- Ability to work with the mentally ill.
- Ability to respond effectively in a wide variety of human service situations.
- Must be at least 21 years of age.
- Must have a current valid driver’s license.
- Must successfully complete Case Management certification as stipulated by the State of Kansas within six months of employment and maintain Case Management Certification thereafter.
- Must pass KBI, SRS, child abuse check, adult abuse registry, and motor vehicle screens.
- Will obtain CPR and First Aid Training and renew training annually.
- Will attend any other training and certifications directly related to this position.
Nice To Haves
- Experience working with persons diagnosed with a Severe and Persistent Mental Illness is preferred.
