About The Position

As a Targeted Case Manager (TCM) at Flourishing Peaceful Families, you will play a vital role in helping children, adolescents, adults, and families access the services and supports necessary to improve their overall well-being. The Targeted Case Manager serves as an advocate, coordinator, and resource navigator, working collaboratively with clients, caregivers, healthcare providers, schools, community agencies, and other stakeholders to ensure clients receive comprehensive and coordinated care. The ideal candidate is highly organized, compassionate, knowledgeable of community resources, and committed to helping individuals overcome barriers to achieving their personal, educational, behavioral health, medical, and social goals.

Requirements

  • Proven experience in case management, social work, or a related field.
  • Strong organizational and communication skills.
  • Familiarity with community resources and services.
  • Ability to work independently and collaboratively within a team.
  • Compassionate and client-centered approach to service delivery.
  • Knowledge of relevant ethical and legal standards in case management.

Responsibilities

  • Complete comprehensive needs assessments to identify client strengths, needs, barriers, and service priorities.
  • Develop, implement, and regularly update individualized care plans in collaboration with clients, families, and treatment team members.
  • Monitor client progress toward identified goals and modify care plans as needed.
  • Coordinate behavioral health, medical, educational, housing, employment, financial, and social services for clients.
  • Facilitate communication among service providers to ensure continuity and effectiveness of care.
  • Advocate for clients to obtain necessary services and resources.
  • Identify and connect clients with community resources, benefits, and support services.
  • Assist clients with applications, referrals, and enrollment processes for eligible programs and services.
  • Provide education regarding available community resources and service options.
  • Maintain regular contact with clients, families, and service providers to monitor service effectiveness and client satisfaction.
  • Evaluate progress toward goals and address barriers that may impede successful outcomes.
  • Ensure clients remain engaged with recommended services and supports.
  • Work closely with therapists, psychiatric providers, schools, child welfare agencies, probation officers, healthcare providers, and other community partners.
  • Participate in interdisciplinary treatment team meetings and case consultations.
  • Support clients in achieving greater independence, stability, and self-sufficiency.
  • Maintain timely, accurate, and complete documentation in accordance with Medicaid, state, federal, and agency requirements.
  • Complete needs assessments, care plans, progress notes, collateral contacts, and other required documentation within established timeframes.
  • Adhere to HIPAA regulations, ethical standards, and all applicable laws governing case management services.
  • Assist clients and families in accessing appropriate crisis intervention and emergency resources when needed.
  • Communicate concerns regarding client safety or risk factors to appropriate parties and supervisors.
