About The Position

Hope Services, LLC is dedicated to providing high-quality, evidence-based Mental Health services to children, adolescents, and families. This full-time salaried leadership role is responsible for providing clinical, administrative, and operational supervision to the Care Management Team while promoting whole-person, integrated care.

Requirements

• Provisionally or fully licensed as an LCSW, LCMHC, LPA, LMFT, or RN.
• At least 3 years of experience providing care management, case management, or care coordination.

Nice To Haves

• Experience working with individuals with co-occurring Behavioral Health and I/DD needs is preferred.

Responsibilities

• Provide clinical and administrative supervision to a care management team consisting of up to 8 Care Managers and 3 Extenders.
• Lead staff recruitment, onboarding, training, performance management, discipline, and termination processes.
• Ensure quality service delivery and compliance with service definitions through activities such as: Facilitating case conferences, Conducting record audits, Observing team performance, Participating in home visits and Child and Family Team (CFT) meetings.
• Ensure timely and accurate documentation of all Care Management activities in accordance with policy and program requirements.
