About The Position

Dixon Social Interactive Services is seeking a compassionate, community-focused Care Manager Extender to support our Tailored Care Management team in helping members navigate complex healthcare, behavioral health, and social service systems. The Care Manager Extender works under the direction and supervision of a Supervisory Care Manager and assists with member engagement, care coordination, resource navigation, and follow-up activities. This position helps expand care management capacity and ensures members receive the support and connections they need to access appropriate services and community resources. The Care Manager Extender does not provide clinical services and must perform all assigned duties within the defined scope of Tailored Care Management functions.

Requirements

  • Must be at least 18 years of age.
  • Must have a high school diploma or equivalent, such as a GED or certificate of completion.
  • Lived experience with an intellectual/developmental disability (I/DD) or traumatic brain injury (TBI); and Demonstrate knowledge of and direct personal experience navigating the NC Medicaid delivery system.
  • Lived experience with a behavioral health condition; and Be a Certified Peer Support Specialist.
  • Be a parent or guardian of an individual with an I/DD, TBI, or behavioral health condition; and Have at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual.
  • Have at least two years of paid experience performing functions similar to those described under Care Manager Extender responsibilities; and Have at least one year of paid experience working directly with the Tailored Care Management-eligible population.

Responsibilities

  • Assist with outreach and engagement activities to help members access needed services and supports.
  • Support members in understanding care plans, appointments, referrals, and available services.
  • Provide education and information regarding community resources and Medicaid services.
  • Help identify and address barriers to care, including transportation, scheduling, communication, and access to services.
  • Build positive, respectful relationships with members and support member-centered goals.
  • Assist Care Managers with assigned care coordination and follow-up activities.
  • Document member interactions, contacts, and activities accurately in the care management system.
  • Communicate member needs, concerns, barriers, and updates to the assigned Care Manager.
  • Assist with follow-up related to referrals, appointments, service connections, and community resources.
  • Participate in team meetings and care coordination activities as requested.
  • Connect members with appropriate community-based services, programs, and support resources.
  • Assist members in navigating healthcare, behavioral health, social service, and community systems.
  • Use community knowledge and, when applicable, lived experience to help members identify and access resources.
  • Assist members with social determinants of health needs, including: Housing, Food and nutrition, Employment, Transportation, Healthcare access, Other community and social supports.
  • Work under the direction and supervision of the Care Manager and perform only assigned and delegated functions.
  • Maintain member confidentiality and comply with HIPAA and organizational policies.
  • Ensure all activities remain within the approved Care Manager Extender scope of practice.
  • Complete and maintain all required training in accordance with applicable state and federal requirements.
  • Collaborate effectively with Care Managers, Supervisory Care Managers, and other members of the care team.
