About The Position

The System of Care (SOC) Navigator serves as the central referral hub for MHRC programs and community resources. This position coordinates internal and external referrals, facilitates warm handoffs, addresses barriers to care, and promotes timely access to integrated, person-centered services. A primary focus is ensuring uninsured and underinsured individuals are connected to appropriate services within seven (7) days of discharge, including psychiatric and medication follow-up when needed. The SOC Navigator engages individuals prior to discharge, provides education regarding MHRC’s Comprehensive Services Center (CSC) Central Receiving System, facilitates intake and service connections, and follows up to confirm successful linkage. The SOC Navigator also identifies individuals at risk for repeated acute-care utilization and provides outreach, referral coordination, and continuity-of-care support to help prevent unnecessary crisis episodes, Baker Act admissions, and re-hospitalizations through least-restrictive interventions.
