About The Position

The System of Care Coordinator role focuses on enhancing the System of Care framework and case management for at-risk youth, emerging adults, and families. This position involves expanding school and community collaboration within the serviced region by increasing presence in interagency planning and community support meetings, identifying and nurturing relationships with school staff, developing and operationalizing local social marketing, collaborating with community partners for educational and employment supports, and overseeing a case management team for at-risk youth and emerging adults. The coordinator will also provide training, technical assistance, and workforce development to local system of care partners, and implement a community-wide training curriculum on evidence-based programs. Responsibilities include interviewing clients, conducting diagnostic tests, providing outpatient and/or in-home services (individual and family therapy, group therapy), performing psychosocial and crisis assessments, ANSA, CANS, and developing treatment plans. The role also involves updating service and treatment plans, communicating with referral agencies (DFACS, Juvenile Courts, Schools), collaborating with Community Support Individuals and other mental health professionals, assisting with client personal development and work performance, providing therapeutic interventions and support, intervening in crisis situations, and documenting client progress related to treatment plans. The position requires ensuring programmatic requirements are met based on various oversight/funding entities (DBHDD, CMS, HUD, DCA), providing budget oversight and monitoring, and developing/monitoring programmatic and financial outcomes reports. Additionally, the coordinator must ensure staff are appropriately trained and manage Human Resources aspects in conjunction with the HR Department.

Requirements

  • Masters Degree from an accredited institution in psychology, social work, counseling or marriage or family therapy.
  • License eligible in the state of Georgia.

Responsibilities

  • Enhance the System of Care framework and case management for at-risk youth, emerging adults, and families.

  • Establish a greater presence in local interagency planning meetings and other community support meetings.
  • Identify and nurture key relationships with staff in current school systems.
  • Develop and operationalize local social marketing.
  • Collaborate with community partners for educational and employment supports.
  • Oversee case management team and series for at-risk youth and emerging adults.
  • Provide training, technical assistance and workforce development opportunities to local system of care partners.
  • Implement a community wide training curriculum that would incorporate training on evidence based programs for partner organizations.

  • Perform psychosocial assessments, crisis assessments, ANSA, CANS, and treatment plans.
  • Update individual service and treatment plans.
  • Communicate with various referral agencies including DFACS, Juvenile Courts, and Schools.
  • Work with a team of Community Support Individuals and collaborate with other mental health professionals.
  • Assist with personal development and work performance of the client, and other behavioral, therapeutic interventions and support.
  • Support and intervene in crisis situations.
  • Provide documentation notes that relate to the customer-driven mental health treatment plan.
  • Assure programmatic requirements are met based on various oversights/funding entities: DBHDD, CMS, HUD, DCA.
  • Provide budget oversight & monitoring.
  • Develop & monitoring of programmatic & financial outcomes reports.
  • Assure that staff is appropriately trained and current in training.
  • Provide Human Resources management skills (in conjunction with the HR Department).
