Continuum of Care Coordinator
About The Position
The Continuum of Care Coordinator at Via Christi Home Medical on Washington in Wichita, KS, is responsible for coordinating patient care throughout the healthcare continuum. This full-time, day shift role involves managing communications with patients, families, physicians, and healthcare team members, as well as coordinating patient scheduling and maintaining required documentation. The coordinator also supports standard patient access initiatives, helps patients navigate the healthcare system, and assists with utilizing available community support services. The schedule is Monday-Friday, 8am-5pm, with a rotating on-call component.
Requirements
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Nice To Haves
- Rotating On-Call Schedule: Sat and Sun 8am-5pm - in rotation with staff
Responsibilities
- Provide coordination of patient care throughout the healthcare continuum.
