This role is located at St. Vincent Hospital and Health Care Center in Indianapolis, IN, within the Diagnostic Breast Care department, on a PRN schedule.
Requirements
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Nice To Haves
- Licensed Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board.
- Licensed Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board.
- Rt(R)(M) with MQSA and NCBC experience
Responsibilities
- Provide coordination of communications with patients, families, physicians, and healthcare team members.
- Coordinate patient scheduling with appropriate resources or clinical services and maintains required documentation.
- Support standard patient access initiatives across the healthcare continuum.
- Provide support with navigating the healthcare system and assists patients with utilizing available community support services.
