About The Position

The Surgical Nurse Navigator is an essential member of the surgical care team who serves as a resource and advocate for patients throughout their surgical journey. This role coordinates care, provides patient education and support, and works closely with surgeons and the multidisciplinary healthcare team to ensure a safe, seamless experience from preoperative preparation through postoperative follow-up.

Requirements

  • Current Registered Nurse license in the State of Maine.
  • Associate's degree in Nursing (ADN) or Bachelor's degree in Nursing (BSN) required.
  • BCLS certification required.
  • Strong communication, organization, critical thinking, and patient education skills.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Computer and electronic medical record skills required.
  • Knowledge of medical terminology required.

Nice To Haves

  • BSN preferred.
  • 3–5 years of nursing experience preferred.
  • Surgical, perioperative, care coordination, or clinical office experience preferred.

Responsibilities

  • Serves as a primary point of contact and resource for surgical patients and families.
  • Coordinates patient care from surgical consultation through preoperative preparation, surgery, discharge, and follow-up.
  • Completes nursing assessments and identifies patient needs, concerns, and barriers to care.
  • Coordinates appointments, testing, consultations, clearances, and other preoperative requirements.
  • Provides individualized education regarding surgery, preparation, medications, recovery, and follow-up care.
  • Assesses patient understanding and reinforces education as needed.
  • Communicates patient needs and changes in condition to the appropriate members of the healthcare team.
  • Assists patients and families in accessing appropriate clinical and community resources.
  • Maintains accurate and timely documentation in the electronic medical record.
  • Collaborates with the surgical team to promote quality care, patient safety, and an excellent patient experience.
  • Participates in quality improvement, professional development, and educational activities.
  • Supports and promotes the mission and values of Covenant Health Ministry.
  • Performs other duties as assigned.
