About The Position

Family Service Association of Bucks County is seeking a compassionate and dedicated Nurse Navigator to join their team. This role supports individuals and families facing challenges such as substance use, mental health concerns, homelessness, and other life barriers. The Nurse Navigator will provide advocacy, connect clients to resources, and help them navigate these challenges. The position involves working closely with clients, families, healthcare providers, and community partners to coordinate care, remove barriers to services, and promote health, wellness, and recovery. Services may be provided in various settings including office, home, and community-based locations.

Requirements

  • Valid Pennsylvania LPN or RN license required.
  • Valid driver’s license required.
  • Computer proficiency, including experience with HMIS, Credible, ADP, and Microsoft Office Suite.
  • Strong communication, crisis intervention, problem-solving, organization, and culturally responsive care skills.

Nice To Haves

  • Experience in community health preferred.

Responsibilities

  • Conduct assessments and develop individualized Wellness Plans; coordinate medical, behavioral health, substance use, and community-based services; connect clients to essential resources; and provide ongoing outreach, advocacy, follow-up, and support to promote engagement and continuity of care.
  • Provide nursing support during psychiatric evaluations and medication management appointments, including obtaining vital signs and completing clinical screenings. Assess medical and medication-related concerns, monitor symptoms and medication effectiveness and side effects, and consult with the Psychiatrist as needed.
  • Perform and assist with Medical Assistant duties as needed to ensure coverage and care.
  • Provide education related to medications, diagnoses, wellness, tobacco awareness, harm reduction, and other health-related needs. Collaborate with clients, families, psychiatrists, therapists, nurses, case managers, and community providers to support coordinated care and client goals.
  • Manage medication-related responsibilities, including medication inventory and storage, dispensing oral and injectable medications as prescribed, reviewing PDMP reports, coordinating laboratory and diagnostic testing, completing prior authorizations, and assisting with medication access and insurance needs.
  • Maintain accurate and timely clinical and service documentation; participate in nursing phone line coverage, medical waste procedures, supervision, training, and professional development; maintain required licensure and certifications; and adhere to agency policies, regulatory requirements, and professional standards.
  • Provide services within office, shelter, community-based, home, and offsite locations as needed to serve diverse client populations.
