About The Position

Center for Community Resources (CCR) is seeking a Full-Time Supports Coordinator to serve the Pittsburgh area. CCR has been voted a Best Place to Work in Western PA for 11 consecutive years and is one of the largest Supports Coordination Organizations in Pennsylvania. This role involves working with people with intellectual/developmental disabilities, developing individual support plans, coordinating community living supports, and evaluating progress. Applicants must live in or around Pittsburgh and be willing to drive throughout the county to meet individuals face-to-face.

Requirements

  • Bachelor’s degree with successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services.
  • OR at least 12 college credits in fields listed above AND TWO YEARS’ professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services
  • ACT 33/34 and FBI Clearances, valid driver’s license and proof of car insurance coverage must be provided prior to hire.
  • Must maintain strict confidentiality and be able to exercise sound judgement, be well organized and have excellent time management skills.
  • Computer competency with ability to learn agency software.
  • Knowledge of Community Resources.
  • Able to exercise sound judgment, be well organized, have excellent verbal and written communication skills.
  • Take initiative, be flexible and cooperative.
  • Maintain strict confidentiality.

Nice To Haves

  • At least one year’s experience working with individuals with disabilities preferred.

Responsibilities

  • Use a person-centered approach to develop and maintain the individual support plan.
  • Inform the individual of service and provider choice, assist in the selection of qualified providers.
  • Coordinate community living supports that may include but is not limited to residential, vocational, employment, educational, and social organizations.
  • Offer community activities and support to individuals as well as resources and support to families.
  • Evaluate individual’s progress towards their identified goals and satisfaction of services.
  • Respond to incidents to ensure appropriate actions are taken to protect the well-being, health, safety, and rights of the individual.
