About The Position

This position is responsible for providing supportive services coordination to residents of Wilshire Hills III, a residential housing complex for Seniors, located in Lee's Summit, Missouri. Services provided are designed to be responsive to residents’ individual needs, utilizing the strengths-based model, and will link residents with services provided by our community collaborative partners. The pay is $23-$24 hourly, depending upon qualifications.

Requirements

  • Bachelor’s degree in Social work or degree in Gerontology, Psychology or Counseling is preferred; individuals without a degree but having appropriate work experience are also acceptable.
  • Knowledge of and training in family, elderly and disability services.
  • Knowledge of referral processes.
  • Demonstrate working knowledge of local services for the specific resident profile and people with disabilities.
  • Two (2) to three (3) years of social service delivery experience, preferably direct experience working with families, elderly people or people with disabilities.
  • Superior communication (verbal and written), time management and organizational skills.
  • Comprehend, document and converse in standard English.
  • Superior problem solving and conflict resolution skills.
  • Sound judgment and reasoning skills.
  • Ability to maintain confidentiality.
  • Knowledge of computers and software programs.
  • Ability to handle multiple demands and priorities and work with diverse population.
  • Conversant in and supportive of Catholic Charities’ mission.
  • Ability to consistently meet deadlines and product timely and accurate data/reports.
  • Follows the Catholic Social Teachings in all aspects while performing job duties.

Responsibilities

  • Complete a comprehensive assessment with residents to determine their individual needs and design goals to effectively meet the challenges.
  • Comprehensive written plans must be completed with 72 hours of new resident admission. Individual case plans must be developed with new resident within two (2) weeks of the intake. Plan will include short- and long-term goals with objectives, and plans will be updated as needed.
  • Lead in the development of tasks and activities that will lend itself to a community feeling among the residents.
  • Conduct life skill and budgeting one-on-one or in groups.
  • Coordinate case management efforts with all staff and outside services to meet residents’ needs. Operate as the lead referral agency for residents including coordinating onsite services and referral based social services.
  • Design onsite services, social and physical activities that will be available to the residents.
  • Develop a peer-to-peer mentoring program for the residents and provide oversight of the connections.
  • Maintain a file on each resident that includes eligibility documents, strengths assessment, personal goal plan, case notes documenting action taken, progress, challenges, and follow up. Files should also include all Agency required documentation (e.g., Bill of Rights, Release of Information).
  • Maintain comprehensive statistical data on all residents; submit monthly summary of statistics.
  • Maintain accurate records of financial assistance provided to residents and progress with goals.
  • Maintain internal systems of outcome tracking.
  • Prepare case records for proper storage after resident exits program.
  • Submit expenditure report forms and vouchers monthly.
  • Develop and maintain community collaborations which will support residents’ needs.
  • Work in conjunction with partners to determine eligibility and suitability of potential clients.
  • Stay abreast of current trends and resources available to residents.
  • Attend agency and community meetings as requested.
  • Attend in-service training and/or outside webinars, conferences, or workshops as needed.
  • Complete other reports as requested.
  • Perform other duties as assigned.
